Here are few observations on your proposal;

I would review research question 2 for clarity and to ensure it is not biased
Might you also want to look for what works well- as well as what is not working, which is the current focus?
The methods need another look- as in some places a questionnaire is mentioned, whereas in others there is a clear focus on qualitative research only
The data analysis section can be separated out from the practicalities and some references added to support. If a questionnaire and document analysis are also being considered, these also need to be addressed here.
It would be helpful to add a short section on practicalities and ethics – such as how you would access the hospitals, the permissions needed, how you inform and protect participants etc
The timeline can be reviewed- as a PhD usually takes 3-4 years
I recommend a thorough proofread before submitting
